Prognosis is poor in children with severe idiopathic pulmonary arterial hypertension who present with progressive downhill course. We report the use of oral sildenafil in a 13-year-old girl with suprasystemic idiopathic pulmonary arterial pulmonary hypertension. This therapy improves her quality of life and optimizes the chance of successful lung transplantation.
